Important information – Management of your GP practice is to Change.

If you are a patient registered with Five Elms Medical Centre, Five Elms Lane, Dagenham RM9 5TT, this is to inform you that the management of the practice will change from Monday 4th April 2022.

As you may be aware, Dr Abaniwo has been absent for some time due to illness. In light of his ongoing health issues he has taken the very difficult decision to retire.

Our priority is to ensure patients registered at the practice continue to have access to Primary Care services without interruption.

From Monday 4th April, Omnes Healthcare Ltd will take over management of the practice and the continuing delivery of clinical services until the future of the practice can be determined by the CCG.

Omnes Healthcare Ltd already provide services locally at Porters Avenue and aim to make immediate improvements to services for patients. They will aim to create a ‘Patient Participation Group’ to ensure that patients are involved in service delivery.

There are no intended imposed changes to the staff currently working at the practice.  This means the continuity of your care should not be affected. Recruitment for additional clinical and administrative staff by Omnes Healthcare Ltd is in progress.

We will keep you updated as and when things develop from the above. We understand some patients may have questions or concerns. Please do raise these directly with the practice team at the surgery on 020 8517 1175.
